CASE PLANNER SUPERVISOR
Credential requirement: LMSW or LMHC or LPC

Salary: $70,000

About I’RAISE:
I’RAISE is a proud BIPOC women-led non-profit organization dedicated to the wellness of students in under-resourced communities and public schools in New York City. We implement our Integrative Mental Wellness Model (IMW) at the schools to provide mental health counseling and social work services within public school settings. Additionally, I’RAISE promotes wellness through academic enrichment initiatives, including the Afterschool Program, Gen-Z College & Career Pathway Program, and Genesis Academy Program, which encompasses tutoring, mentoring, and coaching.
Responsibilities:
Hold LMSW, LPC, LMHC License or higher (LCSW or advanced certification preferred)
Supervision and Oversight:
Provide direct supervision to case planners, offering guidance and feedback on case management strategies, interventions, and service delivery.
Review and approve individualized service plans, assessments, and case documentation to ensure accuracy, thoroughness, and alignment with agency goals and regulatory standards.
Monitor caseloads to ensure that case planners maintain manageable workloads, allowing for effective support to families and professional development for staff.
Facilitate regular supervision meetings, case conferences, and group trainings to enhance the professional growth of staff and improve service delivery.
Program Management:
Collaborate with program leadership to ensure that prevention services for high-risk families and ACS-mandated cases are delivered efficiently and effectively.
Work closely with agency leadership to evaluate program performance, identify areas for improvement, and implement new strategies to enhance service delivery.
Assist in the development of policies and procedures to guide case management practices, ensuring compliance with all applicable laws and regulations.
Quality Assurance:
Conduct regular audits of case files, assessments, and service plans to ensure adherence to agency standards and regulatory guidelines.
Provide feedback and coaching to case planners based on file reviews, client outcomes, and field observations.
Lead efforts to maintain a culture of continuous improvement, working with case planners to implement corrective actions where needed.
Crisis Management and Problem Solving:
Assist case planners in managing complex or high-risk cases, providing clinical expertise and support in situations requiring immediate intervention or escalated care.
Serve as a point of contact for case planners in resolving conflicts, facilitating school or community collaboration, and managing client crises.
Collaboration with External Partners:
Serve as a liaison between the agency, school staff, and community partners to ensure that services are well-coordinated and meet the needs of clients.
Represent the program in meetings with external stakeholders, including ACS, school personnel, and other service providers, to address systemic issues and advocate for families.
Compliance and Reporting:
Ensure that all documentation and case records are completed in accordance with district, state, and federal guidelines.
Support the program in meeting all compliance requirements, including timely reporting of service outcomes and client progress.
Staff Development:
Provide ongoing training and professional development opportunities to case planners, focusing on enhancing clinical skills, cultural competence, and service effectiveness.
Mentor and support less experienced staff, fostering a collaborative and learning-oriented team environment.
Additional Responsibilities:
Take on additional duties and responsibilities as assigned by the program leadership.
Actively participate in agency-wide initiatives aimed at improving service quality and outcomes for families.
Qualifications / Requirements:
Master’s degree in Social Work (MSW), Counseling, Psychology, or a related field.
Relevant licensure (LCSW, LPC, or LMHC) preferred.supervisory position.
Minimum of 3-5 years of experience in case management, family support, or mental health services, particularly with high-risk families.
2+ years of supervision experience (supervising social workers and case planners)
Experience working with school systems and knowledge of educational resources and support services for students.
Experience conducting individual and group counseling sessions with students and families.
Excellent communication, interpersonal, and organizational skills
Proficiency in Google Suite and case management software preferred
Travel Requirement:
Ability to travel within New York City public schools to perform job-based duties
